National Identity Card application system. This is a service that enables a National Identity Card applicant (Citizen or Resident Alien) to fill out the National Identity Card application form electronically from anywhere. After filling out the form, the applicant will be required to print it and submit it along with other documents to the nearest NIDA Office (district) for completion of biometric registration.
MANDATORY CITIZEN DOCUMENTS REQUIRED WHEN APPLYING FOR A NATIONAL IDENTITY CARD
- Applicant’s birth certificate
- Copy of birth certificate/affidavit of one parent.
- If the applicant’s citizenship is hereditary: verify with a copy of the birth certificate or a copy of the National Identity Card of one of the parents.
- If the applicant’s citizenship is by registration, verify with a copy of the applicant’s registration citizenship (Dossier Number).
